As the founder, I am passionate about supporting those facing challenging circumstances, guiding teenagers toward the right path, and helping individuals improve their current situation through meaningful career opportunities. I personally review every scholarship submission to ensure each applicant receives thoughtful consideration. My goal is not only to provide education, but to open doors and empower individuals to succeed. Scholarship opportunities are limited. Community Assistance Scholarship
The Jacobs Medical Academy Community Assistance Scholarship is designed to support individuals facing financial hardship who are committed to improving their future through career training.

This scholarship is currently available for the Phlebotomy Technician Program.
Who Qualifies
Priority is given to individuals who meet one or more of the following:
• Women in domestic violence situations
• Experiencing homeless
• Low-income individuals

Required Proof of Financial Needs
Applicants must submit:
• Current Year Tax Return (Required)
AND
• One of the following:
• SNAP / Food Stamp Approval Letter
• Low Income Housing Verification Letter
• Section 8 Housing Letter
• Medicaid Approval Letter
• SSI / Disability Award Letter
• Unemployment Benefits Letter
• TANF Benefits Letter
Note: Documentation must be from the current year.
Scholarship Details
• Reduced tuition opportunity
• Limited availability
• Must be committed to completing program
• Tuition must be paid before class start
• Scholarship is non-transferable

• Scholarship Request Letter: Explaining your situation, why you want to enter healthcare, and how completing the program will help improve your future.

• This scholarship covers $1,146 of the $1,446 tuition. Participant must be ready to pay $300 if selected to cover the remaining balance.

Opportunity Scholarship
(High School Seniors)
The Jacobs Medical Academy Opportunity Scholarship was created to support graduating high school seniors who demonstrate academic achievement, financial need, and a strong interest in pursuing a career in healthcare.
Each application is personally reviewed by the owner of Jacobs Medical Academy. Scholarship availability is limited.

Eligibility Requirements
​
Applicants must:
• Be a graduating high school senior (12th grade)
• Meet GPA requirements for scholarship tier
• Meet low-income requirements
• Have no disciplinary actions during the current school year
• Plan to enroll within 3 months of graduation

Student Essay explaining career goals, program of interest, and future plans

Parent or Guardian Letter explaining financial considerations and need

School Counselor Verification Form (form can be downloaded on this page)

Drivers License, Permit or ID

Low-Income Verification (submit one of the following):
• Free or Reduced Lunch eligibility letter
• SNAP or Food Stamp benefits letter
• Medicaid card or approval letter
• Housing Authority or Section 8 verification
• Government assistance verification (TANF, SSI, etc.)
• Parent income verification such as W-2 or tax return

A school counselor or school official must complete the School Counselor Verification Form. Please download the form below, print it, and take it to your school counselor or school official for completion.
